If you serve as a medical director in a Georgia skilled nursing facility, surveyors are now looking at your role more closely than ever. CMS's revised surveyor guidance, effective March 24, 2025, expanded expectations under F841 (Responsibilities of the Medical Director, 42 CFR §483.70(g)) — and Georgia surveys are applying it.
What F841 requires
Every facility must designate a physician medical director responsible for implementing resident care policies and coordinating medical care. The revised guidance makes clear this is not a figurehead role. Surveyors now expect documented evidence that the medical director is:
- Driving resident care policies — recommending, developing, and approving policies on diagnosis, prescribing, and clinical standards of care
- Participating in the QAA committee — personally or through a formally assigned designee
- Intervening with practitioners whose care is inconsistent with current professional standards, including documentation that the intervention occurred
- Actively involved in the facility assessment — the document that now anchors staffing compliance following the staffing rule repeal
The psychotropic spotlight
The 2025 updates put particular emphasis on psychiatric diagnoses and psychotropic medication oversight. Surveyors are scrutinizing schizophrenia coding, the clinical basis for psychotropic prescribing, and whether gradual dose reduction and non-pharmacologic approaches are being attempted. Medical directors are expected to lead facility policy here — not just sign off on it.

Practical steps for Georgia medical directors
- Get your medical director agreement in writing with duties that mirror the F841 guidance
- Keep minutes showing your QAA participation and policy input — if it isn't documented, it didn't happen
- Audit your facility's psychotropic use and schizophrenia diagnoses before the surveyors do
- Review the facility assessment annually and sign it
